The brief

The Rapid Response Service will facilitate the safe and effective discharge of service users from hospital who have been declared as medically fit for discharge but who may still have care needs that can be met in the service users own home.

Additionally, the service will support people at risk of unplanned hospital admission, carer breakdown and crisis/ high risk cases.

Successful service providers will also be expected to provide support to Service Users with complex health needs.

Following a period of support via the Rapid Response, service users transfer either to a long term package via one of the other contractual arrangements - Care at Home or Complex Care and Support - or no longer require a long term package of care.
