Lot 1: Primary Care Led Support- Deliver support in a primary care setting to people facing Multiple Disadvantage, in Rochdale, Oldham and Manchester.
This high-quality health and social support will be embedded in primary care and work collaboratively with other agencies to improve public health outcomes Total estimated value over 5 years is £2,762,767.81 Lot 2: Palliative Care Support- The provision of a palliative care service for adults experiencing homelessness alongside life-limiting and end-of-life physical health conditions.
