This is notice of an Awarded Contract.

Staffordshire County Council (SCC) procured a contract for a Shared Lives Service, which is a service whereby individuals with assessed eligible social care needs share the family home of a specially recruited and trained Shared Lives Carer and their family.

The objective is to provide the best possible support in accordance with the lifestyle and wishes of each Individual, incorporating a person-centered, strength and asset-based approach to service delivery.

The Shared Lives Service supports a range of Individuals with varying levels of need.

The Service will provide long term Placements, respite (short breaks), opportunities in the day and evening in four or eight-hour sessions, Support Carers, Emergency Placements within the Service and a Home from Hospital Service.

The Provider is appropriately registered with the Care Quality Commission (CQC) to deliver regulated activities in Shared Lives Services.

The Contract term will be from 1st April 2024 to 31st March 2027 with an additional option to extend by up to 2x12months - 1st April 2027 up to 31st March 2028 and 1st April 2028 up to 31st March 2029.
