The brief

Inclusion Health Services: Co-occurring Conditions Support Services SERVICE MODEL: Directly support or signpost people experiencing multiple disadvantage.

Multiple Disadvantage is defined as adults experiencing a combination of - homelessness, substance misuse, mental ill health, domestic abuse and contact with the criminal justice system.

For the purposes of this contract, the focus for multiple disadvantage will be people experiencing co-occurring homelessness, substance misuse and mental ill health.

The service will deliver an equivalent, or enhanced staffing structure and coverage to current arrangements with Local Authorities and existing programmes (i.e.

RSPARG Local Authorities, Greater Manchester Housing First and RSAP as well as Individual Local Authority arrangements i.e.

Manchester and Bolton).

The service is expected to breakdown individual and workforce barriers within the contract i.e. joined up service and support across what was previously RSPARG Local Authorities, Greater Manchester Housing First and RSAP and Manchester City Council.

Integrate existing GMCA Homelessness areas and programmes of delivery within the Enhanced Live Well offer There are a number of similar services and different providers supporting people experiencing homelessness, co-occurring conditions and multiple disadvantage operating across Greater Manchester Providers are expected to strategically collaborate with other providers of Inclusion Health support and identify opportunities to jointly support the workforce in line with Enhanced Live Well and integrated neighbourhood approaches across Greater Manchester.

Providers are expected to take on a Lead Provider role for working collaboratively and building partnerships within this contract (Lot 1 - GP Primary Care and Lot 2 - Palliative Care Service) and within the wider Inclusion Health system across Greater Manchester The number of people supported by the service each year is reflected in professional caseload and number of referrals accepted to be taken forward for direct treatment and interventions.

We expect a much wider cohort of people experiencing rough sleeping and co-occurring conditions (multiple disadvantage) to be supported through regular attendance at drop-ins and day centres and engagement with other professionals through the collaborative approach outlined above.
