The brief

Over the next 3 years (with possible two years extension) our vision is to provide a safe, patient-centred, cost-effective and integrated service that meets the needs of the local population affected by MSK and pain conditions.

We need to improve patient care and to manage growing care needs within the current service.

This initiative aims to: • Improve GP Education and training around assessment and treatment of MSK and Chronic Pain Conditions • Improve Patient Education and increase self-management tools around MSK and Chronic Pain Conditions • Integrate MSK and Pain Services so patient care is better coordinated and managed throughout the patient journey • Expand and integrate community options to support treatment of MSK and Chronic Pain conditions, including physical and psychological lifestyle interventions Encourage and support practices to reduce variation and improve the quality of GP referrals to secondary care Develop stronger links between primary and secondary care The integrated MSK and Pain service will need to help support our planned care strategy to ensure appropriate care is given, often keeping treatment out of hospital and closer to home, so we will need to significantly increase the capacity of the community options component by taking a greater focus on physiotherapy, a single referral access point enabling multi-disciplinary teams to triage and treat as well as an emphasis on psychological/behavioural input to manage pain.

This is a Prime Contract hence the prime contractor remains responsible to the commissioner for the delivery of the entire service, and for the co-ordination of its ‘supply chain’ (i.e. its sub-contractor providers) ensuring that it can and does deliver the entire service.

This contract baseline is for three years (£4,369,175- activity and outcome achieved) with two years possible contract extension- amounting to five years total contract life span (£7,334,120- activity and outcome achieved).
