"This Service seeks to provide accommodation and associated support for families who are homeless and have support needs which are impacting on their ability to maintain or access accommodation.

The intended target group includes those who demonstrate a range of support needs including mental health, substance misuse, a history of offending behaviour and other diagnosed and undiagnosed conditions and/or other challenging behaviour.

The Service Provider will provide short term resettlement support for service users making a final exit from any supported accommodation to independent non -supported move on accommodation.

This service will be provided for up 12 families who are homeless, or at risk of homelessness, across the Hyndburn, Pendle, Rossendale and Burnley district council areas.

In order to ensure that the provision is appropriately dispersed, 3 properties must be provided within each of the 4 districts.
