The Service will include adults who have been assessed and who have eligible needs associated with older age, sensory impairment, learning disability, physical disability, physical ill health, and mental health conditions including dementia. The Provider will determine which client groups their service offer relates to. The primary aim of the Service will be to support with individuals with a learning disability or older people, including people with dementia. The purpose of the Service is to enable people attending community-based daytime opportunities to lead a full and active life and to meet the needs of people holistically.
The Service should promote independence, quality and value for money. The Tender is a joint procurement with the NHS West Yorkshire Integrated Care Board (ICB), to be delivered under 1 single Lot only. Where a service is 100% ICB funded, this Service will be provided under the ICB contract terms and conditions (see NHS Statement of Particulars (Contract) - uploaded with the tender documents). In any other case, the Service will be provided under the Council's contract terms and conditions.
